第十五屆中國（）檢驗醫(yī)學暨輸血儀器試劑博覽會健侖體驗

1．腸熱癥：是傷寒病和副傷寒病的總稱，主要由傷寒桿菌和甲、乙、丙型副傷寒桿菌引起。
典型傷寒病的病程較長。細菌到達小腸后，穿過腸粘膜上皮細胞侵入腸壁淋巴組織，經(jīng)淋巴管至腸系膜淋巴結及其他淋巴組織并在其中繁殖，經(jīng)胸導管進入血流，引起*次菌血癥。此時相當病程的第1周，稱前驅期。病人有發(fā)熱、全身不適、乏力等。細菌隨血流至骨髓、肝、脾、腎、膽囊、皮膚等并在其中繁殖，被臟器中吞噬細胞吞噬的細菌再次進入血流，引起第二次菌血癥。此期癥狀明顯，相當于病程的第2-3周，病人持續(xù)高熱，相對緩脈，肝脾腫大及全身中毒癥狀，部分病例皮膚出現(xiàn)玫瑰疹。存于膽囊中的細菌隨膽汁排至腸道，一部分隨糞便排出體外。部分菌可再次侵入腸壁淋巴組織，出現(xiàn)超敏反應，引起局部壞死和潰瘍，嚴重者發(fā)生腸出血和腸穿孔。腎臟中的細菌可隨尿排出。第4周進入恢復期，患者逐漸康復。
典型傷寒的病程約3～4周。病愈后部分患者可自糞便或尿液繼續(xù)排菌3周至3個月，稱恢復期帶菌者。約有3%的傷寒患者成為慢性帶菌者。
1. Typhoid fever: It is a general term for typhoid fever and paratyphoid fever. It is mainly caused by Salmonella typhi and A, B, C paratyphoid strains.
The typical course of typhoid disease is longer. After the bacteria reach the small intestine, they pass through the intestinal mucosa epithelial cells to invade the intestinal lymphoid tissues, and then to the mesenteric lymph nodes and other lymphoid tissues through the lymphatic vessels and multiply in them. The thoracic ducts enter the bloodstream and cause the first bacteremia. At this time, the first week of the course of disease is called prodromal phase. The patient has fever, general malaise, and fatigue. Bacteria flow with the blood to the bone marrow, liver, spleen, kidney, gallbladder, skin, etc. and multiply there. The bacteria phagocytosed by the phagocytic cells in the organs enter the bloodstream again, causing a second bacteremia. This period of symptoms is obvious, equivalent to the first 2-3 weeks of the disease, the patient sustained high fever, relatively slow pulse, hepatosplenomegaly and systemic symptoms of poisoning, some cases of skin rose rash. Bacteria stored in the gallbladder are discharged into the intestine along with bile, and part of it is excreted in the feces. Some bacteria can invade the intestinal lymphoid tissue again, causing hypersensitivity reactions, causing local necrosis and ulceration, severe intestinal bleeding and intestinal perforation. Bacteria in the kidneys can be excreted in the urine. In the recovery period of the fourth week, the patient gradually recovered.
Typical typhoid fever takes about 3 to 4 weeks. After recovery, some patients may continue to excrete bacteria from the feces or urine for 3 weeks to 3 months, and they are called carriers of the recovery period. About 3% of typhoid patients become chronic carriers.
