【什么是質(zhì)控品】

1. 質(zhì)控品的來(lái)源： 
  質(zhì)控品的來(lái)源同校準(zhǔn)品大致相同，廠商可能會(huì)更具自己的要求添加了很多物質(zhì)，此時(shí)有些物質(zhì)的添加量常常達(dá)到病理狀態(tài)的高濃度，在應(yīng)用于某一項(xiàng)目時(shí)，對(duì)這個(gè)項(xiàng)目來(lái)說(shuō)基質(zhì)效應(yīng)將更大。

2. 定值方法： 
  有些廠商會(huì)給自己的標(biāo)準(zhǔn)品定一個(gè)定值范圍，這個(gè)定值范圍是由廠商聯(lián)合幾家使用同樣檢測(cè)系統(tǒng)的臨床用戶，僅多次測(cè)定得出的均值。此時(shí)如果將該質(zhì)控品應(yīng)用于另一個(gè)檢測(cè)系統(tǒng)，由于方法學(xué)的不同，可能得出同廠商給出值有較大差異的值。此時(shí)不能認(rèn)為該檢測(cè)系統(tǒng)的準(zhǔn)確度不佳。此時(shí)需要強(qiáng)調(diào)的是檢測(cè)系統(tǒng)都是用來(lái)測(cè)定新鮮血清的，不是用來(lái)測(cè)定質(zhì)控品或其他物質(zhì)的。檢測(cè)系統(tǒng)只有在檢測(cè)新鮮血清是得出的結(jié)果才具有溯源性。不同檢測(cè)系統(tǒng)之間只有在檢測(cè)新鮮血清時(shí)才具可比性。

檢測(cè)抗原 系用免疫學(xué)方法檢測(cè)宿主細(xì)胞內(nèi)的病原(速殖子或包囊)、在血清及體液中的代謝或裂解產(chǎn)物(循環(huán)抗原)。是早期診斷和確診的可靠方法。國(guó)內(nèi)外學(xué)者建立了McAb-ELISA以及McAb與多抗的夾心型ELISA法檢測(cè)急性患者血清循環(huán)抗原，其敏感度為能檢出血清中.μg/ml的抗原。[] 皮內(nèi)試驗(yàn)以受染小白鼠腹腔液或雞胚液作抗原。常出現(xiàn)延遲性、結(jié)核菌素反應(yīng)。可用作流行病學(xué)調(diào)查。目前應(yīng)用不多。[] 準(zhǔn)媽媽的孕期體檢中，包括一項(xiàng)叫做TORCH的檢查。TORCH一詞是由幾種病原體英文名稱的*個(gè)字母組合而成。其中字母T就代表弓形蟲(chóng)。（其他幾個(gè)字母分別代表梅毒、風(fēng)疹病毒、巨細(xì)胞病毒和單純皰疹病毒。）雖然法國(guó)和奧地利等國(guó)家規(guī)定對(duì)孕婦常規(guī)進(jìn)行弓形蟲(chóng)抗體檢查，做法是在懷孕早期進(jìn)行血清抗體檢查，如果為陰性（即沒(méi)有感染過(guò)），告訴孕婦注意預(yù)防感染，并定期復(fù)查；發(fā)現(xiàn)孕婦出現(xiàn)急性感染，即給予螺旋霉素治療，同時(shí)對(duì)胎兒進(jìn)行羊膜穿刺和超聲檢查。由于孕婦在妊娠期間的感染和胎兒感染的機(jī)會(huì)都很小，所以英美的研究結(jié)論是這種篩檢不值得常規(guī)做?？构蜗x(chóng)IgM抗體陽(yáng)性提示感染。
