玻璃體變性，伴隨著玻璃體稠度的變化，即玻璃體液化和凝縮。液化的玻璃體充滿了后脫離后留下的空隙，一旦視網膜裂孔形成，液化玻璃體會由此進入視網膜下腔，當進入液體超過色素上皮向后轉運能力時，液體就逐漸積聚，zui終發(fā)生視網膜脫離。
視網膜脫離與許多因素有關，有些因素極易導致視網膜脫離，因而被稱為易患因素。主要有：①近視眼：近視眼易于產生玻璃體變性及后脫離。視網膜變性如格子樣變性、鋪路石樣變性等更可能在近視眼中見到。近視眼周邊視網膜的脆弱性，又有玻璃體視網膜牽引，很容易導致視網膜脫離。②無晶體眼：白內障手術伴有玻璃體并發(fā)癥的人特別容易發(fā)生視網膜脫離。發(fā)生在白內障囊內摘除術后者可能與填補原晶體空間的玻璃體運動有關。玻璃體腔的大，增加了玻璃體擺動的空間，使其對視網膜的牽引力增強。術中玻璃體的丟失加劇了這種作用。有玻璃體嵌頓時，則改變了玻璃體后脫離的自然狀況，從而誘發(fā)了玻璃體對無晶體眼視網膜的效應。③年齡：老年人玻璃體大多變性、液化，常伴有各種視網膜變性，因而容易發(fā)生視網膜脫離。④視網膜變性：有些視網膜變性，如格子樣變性、霜樣變性、鋪路石樣變性，特別容易形成視網膜裂孔。這是因為變性降低了視網膜的粘著力，抗牽引力降低。⑤外傷：在挫傷中，撞擊運動的瞬間可使眼球暫時變形，盡管眼球壁能順應外力，但玻璃體不能，此時玻璃體基底部與球壁分開，容易產生視網膜鋸齒緣離斷。穿孔性外傷可直接造成視網膜脫離，而后期玻璃體增殖可導致牽引性網脫。 視網膜的脫離是有早期癥狀的，只有早期發(fā)現(xiàn)，才能早期治療。視網膜脫離的早期癥狀有以下幾種。

（1）飛蚊與閃光：出現(xiàn)zui早。實際上是玻璃體后脫離的癥狀。中老年人特別是高度近視眼患者，然后出現(xiàn)大量飛蚊、某一方位持續(xù)閃光時，應警惕視網膜脫離的可能。

（2）中心視力下降：后極部的視網膜脫離，視力急劇下降。周邊部初脫時，對中心視力無影響或影響甚小。同樣在高危病人有視力下降時應詳細檢查眼底。
