【麻疹的介紹】

麻疹病毒腦脊髓炎 : 麻疹(measles)是由麻疹病毒所引起的急性呼吸道傳染病。臨床上以發(fā)熱、咳嗽、鼻炎、 眼結(jié)膜炎、典型的麻疹黏膜斑以及出疹期的特殊斑丘皮疹等為其特征。0.03%～0.2%的麻 疹病人可發(fā)生腦脊髓炎,多見于出疹后1～7天,也有發(fā)生于出疹前及出疹后數(shù)周者,病死率 15%,少數(shù)殘留有后遺癥。麻疹病毒的腦部慢性感染可引起亞急性硬化性全腦炎。 
【病因與病理】
麻疹病毒屬于副黏液病毒科。各地分離出的麻疹病毒抗原性相同,無亞型存在,但近 年來不斷有麻疹病毒的“變異”問題的報(bào)道。日本有關(guān)資料表明,自然界同時存在著不同 血凝素型別的麻疹病毒,從電泳上的泳動速度看,大體可分為三種血凝素:小型(78K)、中 型(80K)及大型(82K),不同型血凝素抗體的保護(hù)作用是否有區(qū)別,尚不清楚。麻疹病毒 經(jīng)組織細(xì)胞培養(yǎng)連續(xù)傳代后,漸喪失其引起黏膜斑、皮疹、發(fā)熱以及免疫性改變等生物學(xué) 變化的能力。 
在電鏡下觀察,麻疹病毒成球狀或絲狀,直徑為120～250nm,中心為核糖核酸和核衣 殼,外有一層脂蛋白包膜,其包膜內(nèi)含有血凝素。常有人胚腎、羊膜、絨猴B淋巴細(xì)胞 B95A細(xì)胞系分離麻疹病毒。 
麻疹病毒對外界環(huán)境抵抗力較弱,在日光下20分鐘即失去致病力,在室溫中24～36 小時即失去活力。含有病毒的飛沫在室內(nèi)空氣中保持傳染性一般不超過2小時。但在 0℃可保持活力達(dá)數(shù)天之久,在干燥、冰凍環(huán)境,可保持活力至少25周。 
麻疹病毒腦脊髓膜炎發(fā)病機(jī)理尚不清楚,可能是麻疹病毒直接侵入中樞神經(jīng)系統(tǒng)或 是麻疹病毒感染后引起的感染后腦脊髓炎即反應(yīng)性腦脊髓炎所致。 
麻疹病毒腦脊髓炎的病理改變可見大腦、丘腦下部、基地節(jié)、腦干、小腦、脊髓出現(xiàn)水 腫、充血、點(diǎn)狀出血、淋巴細(xì)胞浸潤與脫髓鞘改變。

【注意事項(xiàng)】　　

只有全部使用配套試劑才能保證檢測效果，因?yàn)樗性噭┒际怯嘘P(guān)聯(lián)的，不能混用其他制造商的產(chǎn)品。尤其是標(biāo)準(zhǔn)血清，對照血清以及酶標(biāo)記物，必須使用試劑盒配備的，不要使用其它批號產(chǎn)品。稀釋緩沖液，洗液，終止液和底物溶液可用于所有賽潤 ELISA classic試劑盒。 
ELISA classic試劑盒內(nèi)所有試劑均必須正確地存放。應(yīng)在未開封的情況下，保存于2-8℃，并在有效期（見標(biāo)簽說明）內(nèi)使用。詳細(xì)的穩(wěn)定性和儲存資料在“8. 儲存及穩(wěn)定性”內(nèi)將加以詳述。 
每一種試劑都是經(jīng)過校準(zhǔn)以保證*的檢測結(jié)果。這些試劑如果未經(jīng)規(guī)定被稀釋或改動都會導(dǎo)致檢測的敏感性的降低。 
在儲存及孵育過程中避免將試劑暴露在強(qiáng)光中。所有試劑瓶蓋須蓋緊以防止蒸發(fā)和污染，試劑避免受到微生物的污染，因?yàn)榈鞍姿饷傅母蓴_將導(dǎo)致出現(xiàn)錯誤的結(jié)果。 
請從標(biāo)記處剪開密封袋。如果鋁袋破損或者開啟裝有干燥劑的鋁袋后而未以夾子封緊的，則不要繼續(xù)使用微孔條。 
開始試驗(yàn)前，將所有試劑置于室溫。 
從試劑管中吸取試劑的時候，應(yīng)注意使用無菌技術(shù)以防污染。為避免假陽性結(jié)果，吸加酶結(jié)合物時，吸嘴應(yīng)確保不接觸或噴灑于小孔的外面，注意不要蓋錯瓶蓋或管蓋。  
試驗(yàn)結(jié)果的可重復(fù)性依賴于充分混勻試劑。使用前振蕩裝有對照血清的小管和所有稀釋過的溶液（例如使用混合器）。 
小心吸取試劑并嚴(yán)格遵守給定的孵育時間和溫度。請注意在吸取標(biāo)本 / 對照血清，酶結(jié)合物或底物時，*個孔與zui后一個孔加樣之間的時間間隔如果太大，將會導(dǎo)致不同的 “預(yù)孵育”時間，從而明顯地影響到測量值的準(zhǔn)確性及重復(fù)性。 
如果實(shí)驗(yàn)中未嚴(yán)格遵守控制質(zhì)量控制認(rèn)證文件上的有效性特異準(zhǔn)則，則試驗(yàn)結(jié)果無效。 
洗滌不充分將影響試驗(yàn)結(jié)果： 
 
應(yīng)該小心進(jìn)行洗滌。洗滌步驟應(yīng)遵守相關(guān)洗滌器（平底孔，孔直徑7毫米，深10.9毫米）指導(dǎo)手冊進(jìn)行。重要的是所有的孔內(nèi)均應(yīng)加入相同體積的洗滌緩沖液。洗滌結(jié)束時，應(yīng)將微孔板倒扣于紙巾上并輕輕敲打，以確保所有孔內(nèi)均無洗滌緩沖液。避免產(chǎn)生泡沫！如果使用自動洗板機(jī)，注意正確操作。 

【檢測原理】

Measles encephalomyelitis: Measles is an acute respiratory infection caused by the measles virus. Clinically, fever, cough, rhinitis, conjunctivitis, typical measles mucosal plaque and rash of the special spot dumb rash and so on for its characteristics. 0.03% ~ 0.2% measles patients may occur encephalomyelitis, more common in the rash after 1 to 7 days, but also occurred in the rash before and after rash weeks, the case fatality rate of 15%, a small number of residual sequelae. Chronic viral infection of the brain can cause subacute sclerosing pan-encephalitis.
First, the etiology and pathology
The measles virus belongs to the Paramyxoviridae family. The isolated measles virus has the same antigenicity and no subtype. Measles virus resistance to the external environment is weak, 20 minutes in the sunlight that is, loss of virulence, at room temperature in 24 to 36 hours that is, loss of vitality. The droplets containing the virus in indoor air remain infectious generally no more than 2 hours. But at 0 ℃ to maintain vitality for several days, in dry, frozen environment, can maintain vitality for at least 25 weeks.
The pathogenesis of measles virus meningitis is not yet clear, may be caused by the measles virus directly invaded the central nervous system or caused by infection with measles virus encephalomyelitis or allergic encephalomyelitis.
